It investigates how treatment and vaccination strategies influence disease transmission in high-burden regions, with a strong focus on real-world applicability. A detailed multi-stage infection model is developed, incorporating asymptomatic, mild, and severe malaria cases alongside mosquito vector dynamics. The study uniquely compares three fractional operators Caputo, Caputo-Fabrizio, and Atangana-Baleanu-highlighting how memory effects shape disease progression and intervention outcomes. Using real malaria data (2012-2021), the model demonstrates strong predictive accuracy and reveals that integrated control strategies significantly reduce infection levels over time. Results show that vaccination and prompt treatment, when combined with vector control, can effectively suppress malaria outbreaks and improve long-term public health outcomes.
